West Muskingum Schools has partnered with FinalForms, an online forms and data management service that replaces paper forms. FinalForms allows you to complete and sign student enrollment, back-to-school and athletic participation forms for your new or current student. We encourage you to review the Playbook that applies to your student, new or current. Click FinalForms to the right to get started!
You can review and update your data at any time during the school year. You will be required to sign your forms once a year and after any update you may make, such as a change of address or phone number. Registration must be completed by the parent or legal guardian.
For new students, the parent/guardian will be asked to provide the following documentation later in the enrollment process:
- Copy of birth certificate
- Immunization records
- Proof of residency (utility bill, cable bill, etc.)
- Parent/Legal Guardian photo ID
- Divorce/Custody papers (if applicable)
- Current IEP/ETR/504 Plan (if applicable)
If you're unsure if your address is in the West Muskingum School District, you can visit the Muskingum County Auditor's website at http://muskingumcountyauditor.org/ or the Licking County Auditor's website at http://apps.lcounty.com/auditor/OnTrac/. Families living outside of the district, must complete an Open Enrollment application first.
Kindergarten Registration for 2020-2021
Children who will be five years old on or before July 31, 2020, are eligible to register for kindergarten at West Muskingum Elementary School for the 2020-2021 school year. Registration must be completed by the parent or legal guardian. Click on the FinalForms link to complete the student enrollment form.
(Children who will not be five on or before July 31, can be evaluated for possible early entrance to kindergarten. Evaluations must be scheduled through the Licking County Educational Service Center by calling (740) 349-6084.
Families living outside of the West Muskingum School District interested in enrolling their student, must first submit an Open Enrollment application. The 2020-2021 application is now available for any new students. Click on the link under forms to submit your application online. One application per student is required. Once the application is approved you will be asked to begin the FinalForms enrollment process.
West Muskingum Elementary School houses two preschools, which are operated by the Muskingum Valley Educational Service Center. Children who are age three and four as of August 1, are eligible to apply for enrollment. Beginning the 2020-21 school year, classes will be two full days per week, based on the district/building calendar. Flexibility is available to allow families to set part-time schedules upon request. An income-based sliding fee scale is utilized to determine tuition costs. Transportation is usually available through the school district.
